Scope

We have an exciting new opportunity for an experienced Care Coordinator to join the Loveday team. This role involves overseeing the management of a specific floor, department or area reporting directly to the Deputy Manager/Clinical Lead with regards to all areas of resident care. He or she will manage his or her area by example, being visible, involved and accessible at all times. Responsibilities include but are not limited to; training Care Assistants for their department, facilitating induction, dealing with the team member recognition, one-to-one meetings, yearly performance reviews and ongoing coaching and counselling of team members.

 Candidates must have NVQ in Health and Social Care

Key accountabilities:

 

·        Ensuring excellence in member and person centred care,

·        Providing outreach and support to families,

·        Retaining dedicated Care Assistants,

·        Facilitating ongoing training and educational opportunities for staff and families.

·        Have existing knowledge and constantly learning about the most current information on Dementia.

 

Main duties and responsibilities

· Co-ordinate the member assessment process, ensuring involvement of the resident in identifying needs and preferences, and integration of all information into the care plan.

· Ensure that all relevant information in respect of each member is communicated as appropriate to achieve delivery of person-centred approach.

· Champion the organisation’s vision and values, adopting an open and approachable management style supporting person-centred care.

· Actively support and implement effective Infection Prevention and Control measures.

· Complete all allocated one-to-one meetings and yearly performance reviews with staff in the department.

·  Ensure that all staff go through the Loveday induction programme, completing the necessary paperwork and providing evidence of completion  through hands-on monitoring and support.

· Ensure appropriate staff allocations and completion of daily assignments.

· Participate in and facilitate, when appropriate, the company’s training programmes and structured induction programme, ensuring all your staff are well inducted and trained to the necessary standards.

· Regularly communicate to family members with regards to member needs and progress.

· Report staff absence to Deputy Manager/Clinical Lead and complete Return To Work forms

· Administer and safely manage and audit  medication

· Conduct and oversee effective handovers between all shifts, ensuring that the General Manager is kept fully informed at all times with regards to applicable areas.

· Assume responsibility for Health and Safety compliance within your specific area at the home.

· Provide daily coaching and support to Care Assistants as needed, leading by example at all times.

· Develop and execute daily programming for members focusing on person centred activities.

 

Health and Safety:

The post-holder will maintain their personal responsibilities under Health and Safety at Work Regulations, and compliance with the company Health and Safety Policy, ensuring safe working practices and the recommended storage of equipment, including but not exclusive to:

· Control of Substances Hazardous to Health, Electricity at Work, Lift & Lifting Equipment, Manual/Object Protective Equipment, Legionella/Water Treatment, Waste Management, Reporting of Injuries, Diseases & Dangerous Occurrences Regulations.

· Appraise and assess measures used to prevent infection control hazards, taking into account the individual nursing, social, physical and psychological needs of the member.

· Follow and report all concerns and incidents in line with the company incident reporting and whistleblowing procedure, including safeguarding adults’ procedures.
